I think it was fake. The dude just randomly has a camera going? Why doesn't the upset guy notice his bro put a camera there? Who would actually put a mote up their butt? Well we clearly see the brother hide it at the beginning, and we don't know where he put it. He could have put it on a cluttered ass shelf where it wouldn't have been noticed. I don't know if it's real or fake, but a lot of people are saying while you can fake all the flailing and screaming, it's harder to fake the looks of emotion he had on his face which sells it.
