I've found that toys work the best, really. The easiest way I've found is her on her back, and you in her at an angle. Think upside down T. If she puts her leg over you, or you push it up, it provides pretty easy access to the vag. This allows you to work both at the same time. Missionary and doggy are hard to manage in that fashion, but on your side provides a good amount of breathing room to work your shit.
